Professionally trained staff are available to assist you while you are enrolled in our program. We strive to provide a family-like atmosphere, where our greatest priority is the care and wellbeing of our participants.
Foundations Medical Adult Day Care is dedicated to enhancing the quality of life for each participant. Our professional staff understands that each person has his/her own distinct needs. Our staff is made up of Registered Nurses, Licensed Practical Nurses, Social Workers, Nursing Assistants, and other types of health care professionals.
These staff members are capable of serving a wide variety of participants. Prior to placement in our program, each potential participant is evaluated to ensure that our program will be appropriate and beneficial.
The program is open 7 days a week and provides transporation to and from home. We also, if feasible, provide transportation to physician's offices and other types of appointments.

CRITERIA for ADMISSION
Foundations Medical Adult Day Care is licensed by the Office of Health Care Quality and has been deemed eligible to participate in Maryland's Medical Assitance program.
Foundations accepts Medical Assistance, some third party insurances, and private pay as payment for services.
Community Based Admissions
Individuals with Medical Assistance must be deemed eligible by the State to receive Medical Adult Day Care services. The eligibility process is initiated by interested parties, patients, families, or caregivers. Any of these individuals should call their local Adult Evaluation and Review Service Office. This phone call will initiate the eligibility/assessment process.

Hospital or Inpatient Admissions
Social Workers, Discharge Planners, and other staff based in hospitals, or other inpatient settings, wishing to refer Medical Assistance patients to Foundations must initiate the eligibility process by submitting the 3871B to Delmarva Foundation.
On July 1, 2008, the Medical Adult Day Care program was converted from a State Plan to a Waiver Plan. As a result of this conversion, Medical Adult Day Care providers are no longer able to initiate the eligibility process.
